BackgroundAlthough patient advocates have developed templates for standard consent forms, evaluating patient preferences for first in human (FIH) and window of opportunity (Window) trial consent forms is critical due to their unique risks. FIH trials are the initial use of a novel compound in study participants. In contrast, Window trials give an investigational agent over a fixed duration to treatment naïve patients in the time between diagnosis and standard of care (SOC) surgery. Our goal was to determine the patient-preferred presentation (...) of important information in consent forms for these trials.MethodsThe study consisted of two phases: (1) analyses of oncology FIH and Window consents; (2) interviews of trial participants. FIH consent forms were analyzed for the location(s) of information stating that the study drug has not been tested in humans (FIH information); Window consents were analyzed for the location(s) of information stating the trial may delay SOC surgery (delay information). Participants were asked about their preferred placement of the information in their own trial’s consent form. The location of information in the consent forms was compared to the participants’ suggestions for placement. Results34 [17 FIH; 17 Window] of 42(81%) cancer patients approached participated. 25 consents [20 FIH; 5 Window] were analyzed. 19/20 FIH consent forms included FIH information, and 4/5 Window consent forms included delay information. 19/20(95%) FIH consent forms contained FIH information in the risks section 12/17(71%) patients preferred the same. Fourteen (82%) patients wanted FIH information in the purpose, but only 5(25%) consents mentioned it there. 9/17(53%) Window patients preferred delay information to be located early in the consent, before the “Risks” section. 3/5(60%) consents did this.ConclusionsDesigning consents that reflect patient preferences more accurately is essential for ethical informed consent; however, a one-size fits all approach will not accurately capture patient preferences. We found that preferences differed for FIH and Window trial consents, though for both, patients preferred key risk information early in the consent. Next steps include determining if FIH and Window consent templates improve understanding. (shrink)

Physical Relativity explores the nature of the distinction at the heart of Einstein's 1905 formulation of his special theory of relativity: that between kinematics and dynamics. Einstein himself became increasingly uncomfortable with this distinction, and with the limitations of what he called the 'principle theory' approach inspired by the logic of thermodynamics. A handful of physicists and philosophers have over the last century likewise expressed doubts about Einstein's treatment of the relativistic behaviour of rigid bodies and clocks in motion in (...) the kinematical part of his great paper, and suggested that the dynamical understanding of length contraction and time dilation intimated by the immediate precursors of Einstein is more fundamental. Harvey Brown both examines and extends these arguments, after giving a careful analysis of key features of the pre-history of relativity theory. He argues furthermore that the geometrization of the theory by Minkowski in 1908 brought illumination, but not a causal explanation of relativistic effects. Finally, Brown tries to show that the dynamical interpretation of special relativity defended in the book is consistent with the role this theory must play as a limiting case of Einstein's 1915 theory of gravity: the general theory of relativity.Appearing in the centennial year of Einstein's celebrated paper on special relativity, Physical Relativity is an unusual, critical examination of the way Einstein formulated his theory. It also examines in detail certain specific historical and conceptual issues that have long given rise to debate in both special and general relativity theory, such as the conventionality of simultaneity, the principle of general covariance, and the consistency or otherwise of the special theory with quantum mechanics. In many research studies, tumor biopsies are an unavoidable requirement for achieving key scientific aims. Yet some commentators view mandatory research biopsies as coercive and suggest they should be optional, or at least optional until further data are obtained regarding their scientific usefulness. Further complicating the ethical picture is the fact that some research biopsies offer a potential for clinical benefit to trial participants. We interviewed and surveyed a convenience sample of participants in phase I clinical trials at a single (...) institution. Our primary aim was to describe phase I participants’ understanding of whether a research biopsy offered them the prospect of medical benefit. We also endeavored to describe participants’ views about biopsies—specifically, the benefits of biopsies, if any, and whether biopsies were acceptable, risky, or discouraged trial participation. Finally, we collected data on demographics and attitudes to see if any strong correlations with misunderstanding, acceptability, or riskiness existed. Overall, the respondents tended to view research biopsies as acceptable, though they did not succeed in identifying the lack of benefit of a research biopsy. These findings call for renewed efforts in consent conversations and documents to carefully describe the benefits, or lack thereof, of research biopsies. (shrink)

The influence of historiography on aspects of political thought in France, Italy and Germany. In recent years the overlap between political thought and historiography has changed the boundaries of intellectual history. Donald Kelley, the longtime editor of The Journal of the History of Ideas has played a leading part in this process. These essays by his friends and former students follow in his footsteps. The collection is divided into three parts: France, England [six essays], and Italy and Germany [four (...) essays]. We review the dynamical approach to spacetime theories---in particular, its origins in the development of special relativity, its opposition to the contemporary `geometrical' approach, and the manner in which it plays out in general relativity. In addition, we demonstrate that the approach is compatible with the `angle bracket school'.

The 'history of ideas', better known these days as intellectual history, is a flourishing field of study which has been the object of much controversy but hardly any historical exploration. This major new work from Donald R. Kelley is the first comprehensive history of intellectual history, tracing the study of the history of thought from ancient, medieval and early modern times, its emergence as the 'history of ideas' in the 18th century, and its subsequent expansion. The point of departure (...) for this study is the perspective opened up by Victor Cousin in the early 19th-century on 'Eclecticism' and its association with the history of philosophy established by Renaissance scholars. Kelley considers a broad range of topics, including the rivalry between 'ideas' and language, the rise of cultural history, the contributions of certain 19th- and 20th-century practitioners of the history of ideas in interdisciplinary areas of philosophy, literature and the sciences, and finally the current state of intellectual history. The central theme of the book is the interplay between the canon of philosophical thought and the tradition of language and textual study, the divergence of the latter marking the 'descent of ideas' into the realm of cultural history. This study reports on the development of scale items derived from the pluralistic moral philosophy literature. In addition, the manner in which individuals combine aspects of the different philosophies in making ethical evaluations was explored.

The conceptual model presented in this article argues that corporations exhibit specific behaviors that signal their true level of moral development. Accordingly, the authors identify five levels of moral development and discuss the dynamics that move corporations from one level to another. Examples of corporate behavior which are indicative of specific stages of moral development are offered.

This paper investigates what the source of time-asymmetry is in thermodynamics, and comments on the question whether a time-symmetric formulation of the Second Law is possible.

Between 1964 and 1990, the notion of nonlocality in Bell's papers underwent a profound change as his nonlocality theorem gradually became detached from quantum mechanics, and referred to wider probabilistic theories involving correlations between separated beables. The proposition that standard quantum mechanics is itself nonlocal became divorced from the Bell theorem per se from 1976 on, although this important point is widely overlooked in the literature. In 1990, the year of his death, Bell would express serious misgivings about the mathematical (...) form of the local causality condition, and leave ill-defined the issue of the consistency between special relativity and violation of the Bell-type inequality. In our view, the significance of the Bell theorem, both in its deterministic and stochastic forms, can only be fully understood by taking into account the fact that a fully Lorentz-covariant version of quantum theory, free of action-at-a-distance, can be articulated in the Everett interpretation. Quantum field theory, one of the most rapidly developing areas of contemporary physics, is full of problems of great theoretical and philosophical interest. This collection of essays is the first systematic exploration of the nature and implications of quantum field theory. The contributors discuss quantum field theory from a wide variety of standpoints, exploring in detail its mathematical structure and metaphysical and methodological implications.

It is argued that awareness of the distinction between dynamical and variational symmetries is crucial to understanding the significance of Noether's 1918 work. Specific attention is paid, by way of a number of striking examples, to Noether's first theorem, which establishes a correlation between dynamical symmetries and conservation principles.

In a comparison of the principles of special relativity and of quantum mechanics, the former theory is marked by its relative economy and apparent explanatory simplicity. A number of theorists have thus been led to search for a small number of postulates - essentially information theoretic in nature - that would play the role in quantum mechanics that the relativity principle and the light postulate jointly play in Einstein's 1905 special relativity theory. The purpose of the present paper is to (...) resist this idea, at least in so far as it is supposed to reveal the fundamental form of the theory. It is argued that the methodology of Einstein's 1905 theory represents a victory of pragmatism over explanatory depth; and that its adoption only made sense in the context of the chaotic state state of physics at the start of the 20th century - as Einstein well knew. One of the widespread confusions concerning the history of the 1887 Michelson-Morley experiment has to do with the initial explanation of this celebrated null result due independently to FitzGerald and Lorentz. In neither case was a strict, longitudinal length contraction hypothesis invoked, as is commonly supposed. Lorentz postulated, particularly in 1895, any one of a certain family of possible deformation effects for rigid bodies in motion, including purely transverse alteration, and expansion as well as contraction; FitzGerald may well have had (...) the same family in mind. A careful analysis of the Michelson-Morley experiment (which reveals a number of serious inadequacies in many text-book treatments) indeed shows that strict contraction is not required. (shrink)

My purpose is to offer an assessment of the scientific legacy of Converse's “Belief Systems” by reviewing five productive lines of research stimulated by his authoritative analysis and unsettling conclusions. First I recount the later life history of Converse's notion of “nonattitudes,” and suggest that as important as nonattitudes are, we should be paying at least as much attention to their opposite: attitudes held with conviction. Second, I argue that the problem of insufficient information that resides at the center of (...) Converse's analysis has not gone away, and that newly fashioned models of information processing offer only partial remedies. Third, I suggest that the concept of the “average voter” is a malicious fiction, as it blinds us to the enormous variation in political attention, interest, and knowledge that characterizes mass publics, in Converse's time as in our own. Fourth, I develop an affirmative aspect of Converse's analysis that has mostly been overlooked: namely, that if ideological reasoning is beyond most citizens’ capacity and interest, they might fall back on a simple and reasonable alternative, which I will call “group‐centrism.” And fifth, I consider the possibility that while the majority of individual citizens falls short of democratic standards, the public as a whole might do rather well. From its first significant appearance in physics, the notion of probability has been linked in the minds of physicists with the notion of uncertainty. But the link may prove to be tenuous, if quantum mechanics, construed in terms of the Everett interpretation, is anything to go by.

Modern insolubility proofs of the measurement problem in quantum mechanics not only differ in their complexity and degree of generality, but also reveal a lack of agreement concerning the fundamental question of what constitutes such a proof. A systematic reworking of the (incomplete) 1970 Fine theorem is presented, which is intended to go some way toward clarifying the issue.

The kinematical principle of Equal Passage Times (EPT) was introduced by Winnie in his 1970 derivation of the relativistic coordinate transformations compatible with arbitrary synchrony conventions in one-dimensional space. In this paper, the claim by Winnie and later Giannoni that EPT is a direct consequence of the relativity principle is questioned. It is shown that EPT, given Einstein's 1905 postulates, is equivalent to the relativistic (synchrony independent) clock retardation principle, and that for standard synchrony it reduces to an isotropy condition (...) for contraction (and dilation) effects. Complementary DNA clones corresponding to most of the proteins of a major amplification and effector of immune host defenses, the complement system, have been isolated and characterized. Availability of these molecular probes has substantially increased our information about and understanding of the structure of the complement proteins and regulation of complement gene expression. Information about the proteins has led to the generation of potential pharmacological agents for the selective control of inflammation. Understanding of the regulatory mechanism has provided insights into (...) the mechanisms accounting for the response to several cytokines including interferon‐gamma, interleukin‐1 and tumor necrosis factor. Finally, complement molecular genetics has been stimulated so that the basis for several complement deficiency disorders has been elucidated. (shrink)

We discuss what we take to be three possible misconceptions in the foundations of general relativity, relating to: the interpretation of the weak equivalence principle and the relationship between gravity and inertia; the connection between gravitational redshift results and spacetime curvature; and the Einstein equivalence principle and the ability to ``transform away" gravity in local inertial coordinate systems.

The Past Hypothesis defended by David Wallace in his 2011 account of macroscopic irreversibility is technically distinct from, but in the same spirit as, that of David Albert in his 2000 book Time and Chance. I am concerned in this essay with the role of objective probability in both accounts, which I find obscure. Most of the analysis will be devoted to the classical treatments by both authors, but a final section will question whether Wallace's quantum version involving unitary dynamics (...) removes this obscurity. We approach the physics of \emph{minimal coupling} in general relativity, demonstrating that in certain circumstances this leads to violations of the \emph{strong equivalence principle}, which states that, in general relativity, the dynamical laws of special relativity can be recovered at a point. We then assess the consequences of this result for the \emph{dynamical perspective on relativity}, finding that potential difficulties presented by such apparent violations of the strong equivalence principle can be overcome. Next, we draw upon our discussion of the (...) dynamical perspective in order to make explicit two `miracles' in the foundations of relativity theory. We close by arguing that the above results afford us insights into the nature of special relativity, and its relation to general relativity. (shrink)

This study blends together ethical philosophy, neurocognitive-evolutionary studies, and literary theory to explore how imaginative discourse addresses a distinctively human deep sociality, and by doing so helps shape cultural and literary history. Deep sociality, arising from an improbable evolutionary history, both entwines and leaves non-reconciled what is felt to be significant for us and what ethical sense seems to call us to acknowledge as significant, independent of ourselves. Ethical Sense and Literary Significance connects literary and cultural history without reducing the (...) literary to a mere expression of something else. It argues that affective differences between non-egocentric and egocentric registers of significance are integral to the bioculturally evolved deep sociality that verbal art addresses-often in unsettling and socially critical ways. Much imaginative discourse, in early societies as well as recent ones, brings ethical sense and literary significance together in ways that reveal their intricate but non-harmonized internal entwinement. Drawing on contemporary scholarship in the humanities and sciences, Donald R. Wehrs explores the implications of interdisciplinary approaches to topics central to a wide range of fields beyond literary studies, including neuroscience, anthropology, phenomenological philosophy, comparative history, and social psychology. Abstract The historical evolution of the principle of relativity from Galileo to Einstein is briefly traced, and purported difficulties with Einstein's formulation of the principle are examined and dismissed. This formulation is then compared to a precise version formulated recently in the geometrical language of spacetime theories. We claim that the recent version is both logically puzzling and fails to capture a crucial physical insight contained in the earlier formulations. The existence of a definite tangent space structure (metric with Lorentzian signature) in the general theory of relativity is the consequence of a fundamental assumption concerning the local validity of special relativity. There is then at the heart of Einstein's theory of gravity an absolute element which depends essentially on a common feature of all the non-gravitational interactions in the world, and which has nothing to do with space-time curvature. Tentative implications of this point for the significance of the vacuum (...) solutions in general relativity, and for the issue of quantising gravity, are briefly examined. (shrink)

The purpose of this paper is to evaluate the `Lorentzian Pedagogy' defended by J.S. Bell in his essay ``How to teach special relativity'', and to explore its consistency with Einstein's thinking from 1905 to 1952. Some remarks are also made in this context on Weyl's philosophy of relativity and his 1918 gauge theory. Finally, it is argued that the Lorentzian pedagogy---which stresses the important connection between kinematics and dynamics---clarifies the role of rods and clocks in general relativity.
